Request a restriction or limitation on the PHI we use or disclose.
5 – Confidential Communications
Request that we communicate with you about medical matters in a certain way or at a certain location.
6 – Copy of this Notice
You have the right to a paper copy of this notice.
V. Changes to This Notice
We reserve the right to change this Notice of Privacy Practices. We will post a copy of the current notice in our facility and on our website.
VI. Changes to This Notice
If you believe your privacy rights have been violated, you may file a complaint with Essential Infusions LLC or with the Secretary of the Department of Health and Human Services. To file a complaint, contact (240) 446-6642. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
